Patients can access a wide range of services included under one annual fee, as well as discounts on most treatment options. This plan features:

Plan Inclusions*
With your plan payment, you will receive these services for free.
- Preventative visits (2/year)
- Teeth cleaning
- Examination by doctor
- Fluoride treatments (1/year)
- Necessary x-rays set (1/year)
*Pedodontic patients pay their standard fees for maintenance less the cost of a standard cleaning.

Total Annual Plan Costs
- Adult membership – $389
- Child membership (under 14) – $339
Guidelines For Savings Plan
- Membership fees are due in full at the time of enrollment.
- Patients are immediately eligible for care at that time.
- Membership payment covers members for 12 months from the enrollment date.
- Any additional charges on treatment are due at the time of service.
- Membership fees are non-refundable.
Limitations For Savings Plan
- This cannot be used as dental insurance or redeemed at other practices.
The discounts and coverages will not be applicable to the following:
- Specialist referrals
- Worker’s compensation related injuries
- Procedures outside the treating doctor’s capabilities
- Hospitalization charges of any kind
- CareCredit or another financing program
- Combine with other promotional offers
